About The Palliative Centre

The Palliative Centre offers a wide range of services in both clinical and community settings to those with a progressive life-limiting illness, alongside practical resources, research and education for patients, families and healthcare professionals.

HammondCare's unique model of care is led by a dedicated team of professionals, including those individuals who are specialists in Palliative Care. This knowledge and experience, enables The Palliative Centre to effectively deliver quality End of Life Care across the aged care and health sector.

HammondCare's mission is “Improving Quality of Life for people in need". The Palliative Centre brings together HammondCare's capability across Palliative Care including; service delivery, research, education and consulting with a specific focus on supporting living well right to the very end "
We are aiming to ensure quality end of life care is available across the aged care sector (residential and home care) through capacity building for all staff to support end of life and palliative care . 

Our palliative care education is practical and user-friendly specifically catering for the care staff workforce utilising resources and research from the National Palliative Care Projects such as  ELDAC, The Advance Project, PEPA, CareSearch and PalliAged.

We are also focused on improving death literacy within the communities we work/serve, including educating family members ensuring they have access to Advance Care Planning information, legislation and educational sessions.

The Palliative Care Bridge delivers innovative educational videos and resources on palliative care by respected experts and specialists in their fields. Our aim is to better equip users of the site to gain confidence and specialised knowledge in the delivery of appropriate palliative care to people in need.

The Advance Project is one of the National Palliative Care Projects, funded by the Australian Government since 2016, and led by HammondCare’s Professor Josephine Clayton working in collaboration with various health organisations and universities across Australia. The Advance Project provides a practical, evidence-based toolkit and a training package, specifically designed to support Australian General Practitioners (GP’s), General Practice Nurses and Practice Managers. The Advance Project enables general practices to implement a team-based approach to initiating advance care planning and palliative care into everyday clinical practice.
The Advance Project has  received further funding from the Australian Government to   adapt some of the existing training programs and practical resources to develop a new suite of dementia-specific advance care planning and palliative care resources for frontline staff working in aged care and primary care.
